Polypharmacy and Structured Medication Reviews project

Supporting polypharmacy in elderly patients

For this key project, we worked with Cambridgeshire and Peterborough ICB  to provide pharmacist support for their large population of patients who were taking multiple medicines (polypharmacy). The requirements of the project were for MMS pharmacists to perform SMRs to help maximise prescribing quality, with a key focus on polypharmacy in the elderly.

Approach

MMS integrated a remote clinical pharmacist team, working with existing embedded pharmacists, to highlight high-risk patients requiring medication reviews and close monitoring – the key aim was to improve clinical outcomes and support patients needing support with long-term conditions. The pharmacist team worked closely with service leads across the ICB, conducting weekly feedback sessions and adding resource and capability to focus on patient cohorts in line with strategic priorities.

Outcomes

The project delivered great outcomes for the ICB, with our pharmacists team able to make a significant impact for the organisation, and importantly, for the patients we reviewed. The seamless integration of our dedicated clinical resource, to work alongside existing pharmacists was a key element to the successful outcomes.
